If you need to change your address (for W-2's) or request a duplicate copy of your W-2, please print out the following forms and fax the completed form(s) to 480-393-4435.

W-2's
NOTE: ALL 2007 W-2's were mailed on Friday, January 25, 2008. All W-2's were mailed to last known home address of each employee. If you have not yet received your W-2, you may not have updated your address. To see if your W-2 has been returned to AER, or to check the address to which we mailed your W-2, please email: juliereyling@aersolutions.com. In the email please include your name, your old address, your current address, your social security number and the client to which you were assigned to work (all to verify identity).
If you send and email to juliereyling@aersolutions.com with the above requested information (name, social security number, old address, current address and client to which you were assigned to work) I will check to see if your W-2 has been returned. If it has been returned I will forward the W-2 to your current address, if it has not been returned I will print out a duplicate W-2 and send to your current address.
Please note if you do not include the requested information (name, social security number, old address, current address and client to which you were assigned to work) in your email, I will not be able to verify your identity and I will not send out a new W-2.
Note you may also print out the Duplicate W-2 Request Form and fax to me at 480-393-4435. Please make sure to sign this form so that your identity may be verified.
401K
If you wish to close out, rollover, or take a distribution from your 401k please download the forms listed below. Please send the signed and completed forms to AER either via fax at 480-393-4435 or via email to juliereyling@aersolutions.com. Please make sure you send the completed forms back to AER, not directly to Hartford. If Hartford receives the forms without the Plan Administers signature, they will be retuned (this will greatly slow down processing time. Once I have received the completed forms I will forward them on to AER's TPA and to Hartford for processing.
